How does it work?
A registered nurse, home health aide or rehabilitation therapist visits the homebound patient in the comfort of his/her home. Treatment is provided by the staff under the direction of the patient's physician.

What are the requirements?
The patient must need skilled nursing or therapy care.
The physician believes the services are necessary and signs an admission order certifying need.
Medicare requires that patients are homebound during the time the skilled care needed.

Home Health Services files Medicare, Medicaid or your private insurance for payment.
What Home Health Services are available to you?
Skilled Nursing - A registered nurse will visit the patient to perform skilled services such as:
Observation and Assessment
Teaching and Training
Obstetrics & Pediatrics
Management & evaluation of a Patient Care Plan
Insertion of catheters and feeding tubes
Obtaining specimens for lab tests
Medical Social Worker - The Medical Social Worker will visit the patient in their home when indicated to:
Locate and contact needed resources
Provide emotional support
Assist patient and/or caregivers in completion of forms/paperwork
Home Health Aide - The home health aide provides personal care for the patient under the direction of a registered nurse, which includes:
Skin, hair and nail care
Toileting and elimination
Exercise and mobility
Reports findings to nurse
Physical Therapy - A physical therapist provides such services as:
Therapeutic exercise programs
Teaching family members to assist patient with exercises
Tests and measurements of range of motion, strength, balance, coordination, endurance and functional ability.
Instructing patient in the use of wheelchairs, braces, crutches, canes and other appliances
Occupational Therapy - Occupational Therapy uses creative and innovative methods by:
Assisting with identifying appropriate adaptive equipment
Assisting patient in returning to maximum level of independence
Teaching patient and family utilization of equipment and therapeutic activities
